WHAT DO YOU NEED FOR YOUR APPOINTMENT:
- Bring your valid Medicare | DVA | Seniors Card
- Bring a list of any medications you are taking
- New patients, please print and fill in a – New patient form or;
- Arrive 10 minutes before your appointment to fill out the necessary paperwork.

Bulk Billing is available for Medicare Card holders All consultations ineligible for bulk billing will incur a fee, including:
